Health Visiting Service
Health visitors deliver the Healthy Child Programme which is designed to offer every family support in making healthy choices. It includes health information, developmental reviews, and access to a range of community services and resources.
If you have any worries about your baby’s or toddler’s development talk to your health visitor, even if it is some time since the last regular visit, or some time since you’ve been to a clinic. Health visitors can come to your home to talk through any concerns you have and can put you in touch with the people who can help if there are problems.

NHS School Aged Immunisation Service
A clinical team offering immunisation programmes to children and young people across Shropshire, Telford and Wrekin, including those that are home educated and in special education schools.
They offer all children the vaccinations they are eligible for depending on their school year or age group. All children and young people are offered their vaccinations at school in the first instance. If your child misses the team when they are at their school or if your child is very anxious they can be offered clinic appointments. Please call or email the team to arrange this.
They offer a slightly longer time for the nurses to spend with children and young people who have additional needs to meet their individual needs and will work closely with those who know your child well to do this.

School Nursing Service
School Nurses are part of the 0-19 team who care for children and young people, aged 5 - 19*, and their families, to ensure their health needs are supported within their school and community. They work closely with education staff and other agencies to support parents, carers and the children and young people, with physical and/or emotional health needs.
*For SEND children the service extends to the age of 25.

Social Prescribing
Telford Mind works in collaboration with Medical Practice across Telford and Wrekin to create a holistic approach to promoting and improving health and wellbeing, adopting a person-centred approach with an ethos of “what matters to me”.
Social Prescribing is a service that is embedded in your GP surgery alongside your GP and other practice staff, that aims to connect you to community services and activities that support your health & wellbeing using a therapeutic approach. It offers:
- A confidential space to talk.
- A space to explore issues that are negatively affecting your physical and mental health and overall wellbeing; those things that ‘Matter To You’.
- Practical, helpful and non judgmental support.
- A place to receive advice, guidance and advocacy
- Support to access services, support organisation and community groups
- Onward referral to support services.
- A service to improve hope for the future, social connection & independence
