SEND One Page Profile or Passport
A one page profile details all the child likes and dislikes in one place, it can provide an ‘at a glance’ profile for the individual child. This is a good way that everyone who comes into contact with the child can see how they can work effectively with the child. For example: If a new practitioner comes into work with the child they can read the profile and see the necessary information on how to best support the child and understand their needs.
The profile or passport should be updated at key transition points or when a significant change occurs. It’s not necessary to update the profile or passport each time an Early Years SEND Support Provision Plan is reviewed/agreed.
This profile or passport can be useful evidence which will be needed if completing and Early Years SEND Inclusion Fund (EY SEND IF) referral or an Educational Health Care Needs Assessment (EHCNA), as appropriate.
Remember this is a way to ensure that the child views are heard, with the help of their parents/carers. It can include pictures, diagrams and can be creative way to share the relevant information.
