Living with a learning disability is different for every person who has one. There are some things that are true for everyone who lives with a learning disability and some common conditions that mean you will have a learning disability.

Creating a One Page Profile

A one-page profile helps professionals know how you would like people to help you, if and when you need care and support.

It’s a summary of your likes and dislikes – it’s all about you!

What can you include in your one page profile?

We asked the SOS Group to share some of the thing they felt were important on one page profiles, here is what they said…

My one page profile talks about what I like

Football, anime, games, watching Netflix, Travel, Cooking

It talks about things I don’t like

Waking up early, Fruit & Veg, mean people, certain foods

What is important you?

Having a career in football, being listened to not being told what to do, my rights, stress, working in large groups, too much pressure

How you like to receive help?

Someone calm who knows what they are doing

Who helps you make decisions?

My mum, parents

Your aspirations and what you want to achieve in life?

To have a career I’m passionate about, to travel the world, be happy, get married, achieve my A Levels, pass my driving test, to become a professional footballer, to play for Manchester United.

What you need to prepare for adult life and live independently?

Learn to cook, gain confidence, improve my communication skills, make friends, get a part time job.

Your one page profile will be unique – but it may change… why?

Because as you get older the way you like to be treated and your aspirations might change so you will have to keep it up to date.

Imagine… would you like to be treated the same at 15 as you were when you were 5?

Have your favourite foods changed as you have grown up, what about your hobbies and interests?

As you get older you will start to think about what job or career you would like and how you would like to live more independently, this can all be included in your one page profile and the About Me section of your Education, Health and Care Plan.
